Terbutaline powder is a medication used to delay preterm labor. It is in a class of drugs called betamimetics, which help prevent and slow contractions of the uterus. Terbutaline is primarily used when doctors need to delay birth for several hours or days in order to allow the child to mature more before being born. The drug can delay labor for up to 48 hours, and since 2011, the FDA has advised that injectable terbutaline should only be used in urgent situations, and that the oral form of the drug should never be used due to treat preterm labor due to ineffectiveness and high risk factors.

04 Terbutaline powder General Description
Terbutaline powder is a relatively selective beta2-adrenergic bronchodilator that has little or no effect on alpha-adrenergic receptors. The drug has exerts a preferential effect on beta2-adrenergic receptors but stimulates beta-adrenergic receptors less selectively than relatively selective beta2-agonists. Terbutaline appears to have a greater stimulating effect on beta-receptors of the bronchial, vascular, and uterine smooth muscles (beta2 receptors) than on the beta-receptors of the heart (beta1 receptors). This drug relaxes smooth muscle and inhibits uterine contractions, but may also cause some cardiostimulatory effects and CNS stimulation.

Terbutaline powder was first indicated and used for the treatment of acute bronchospasm for respiratory conditions such as asthma, bronchitis and emphysema in 1976. Terbutaline belongs to a class of drugs called Beta Adrenergic Receptor Agonists. The anticholinergic properties of the drug can cause dangerous and potentially lethal heart arrhythmias. As indicated for bronchospasm, typically once the acute airway narrowing has been reversed, Terbutaline is stopped and patients are switched to maintenance medications. Terbutaline was never intended for long term use.
Terbutaline became popular for the treatment of preterm labor in the late 1990’s when some cases of preterm labor appeared to respond to the drug. However, Terbutaline has not been efficacious across the board and studies of the drug in injectable, oral and even continuous infusion have shown no efficacy. Terbutaline, a vasoconstrictor, was thought to slow and subsequently halt contractions. However, no matter what the “supposed” effect, there is no medical evidence that Terbutaline, whether injected or taken orally, does anything to halt preterm labor. In 2008, The FDA issued a “Dear Colleague” response to a citizen’s petition, yet it is still being used for the treatment of preterm labor despite its potential for serious and even fatal cardiac complications. To date, 16 maternal deaths can be attributed to cardiac arrest as a result of Terbutaline administration.

Terbutaline is derived from a hormone called epinephrine, which is released when someone is under stress. This response is part of the fight-or-flight response. Stress causes many of the muscles in the body to contract so that a person is ready to respond quickly. However, there are certain muscles that relax instead of contracting during times of stress. Smooth muscle is one type of muscle that relaxes when someone is under stress. Since most of a woman’s uterus is made up of smooth muscle, the uterus will relax in response to a drug that contains certain substances, such as epinephrine.

Terbutaline is used to treat wheezing and shortness of breath from lung problems (e.g., asthma, chronic obstructive pulmonary disease, bronchitis and emphysema). Controlling these symptoms can decrease time lost from work or school. Terbutaline is a bronchodilator (beta-2 receptor agonist) that works by opening breathing passages to make breathing easier.

Terbutaline Powder for Medical uses
Terbutaline is used as a fast-acting bronchodilator (often used as a short-term asthma treatment) and as a tocolytic to delay premature labor. The inhaled form of terbutaline starts working within 15 minutes and can last up to 6 hours.
Terbutaline as a treatment for premature labor is an off-label use not approved by the FDA. It is a pregnancy category C medication and is routinely prescribed to stop contractions. After successful intravenous tocolysis, little evidence exists that oral terbutaline is effective. However, following uterine inversion in the third stage of labor, terbutaline (or either Halothane or magnesium sulfate) can be used to relax the uterus if necessary prior to uterine replacement.
